FOR: Fellows of the Philippine Heart Association
1. LOYALTY AWARD
This award is given to a Past President of the PHA or any of its Chapters who has not been a member of the Board of Directors for the last five (5) years. He must have continuously served the Association in various capacities and has contributed to the furtherance of its goals like :
a. Regular attendance in annual convention and business meetings.
b. Active involvement in the various activities of the Association and continues to be an exemplary source of support and inspiration for the ideals and thrusts of the Association as:
• Speaker - Committee Member, Resource Person
• Lay activities - Fund Raising Campaigns
• or any outstanding activity related to the Association
2. DISTINGUISHED SERVICE AWARD
This award is given to any physician, scientist, lay person or institution who by individual / concerted effort, has made profound contributions to cardiology through delivery of services and the furtherance of the goals of the Association in the national or international level for at least ten (10) years.
3. DISTINGUISHED TEACHER AWARD
This award is given to a Fellow of the college who, because of his/her demonstrated abilities as outstanding teacher, is considered to have made major contributions to cardiology through the teaching and mentoring of students at the undergraduate/post doctoral levels. At least two (2) of the following criteria must be fulfilled:
a. Member of the academe for at least ten (10) years with a rank of assistant professor or higher.
b. Active member of cardiology section of a duly accredited institution by the PHA.
c. Contributor or editor of books in cardiology.
d. Recognized as outstanding teacher in the institution where she/he belongs.
4. DISTINGUISHED SCIENTIST AWARD
This award is given to an individual who:
a. Should have published original papers in reputable local and/or international journals.
b. May have contributed significant scientific knowledge in the field of cardiovascular diseases.
c. May have pioneered in diagnostic and therapeutic procedures in the management of cardiovascular disease.
5. DISTINGUISHED FELLOW AWARD
This is the most prestigious and highest among the College Awards and is conferred to a Fellow of the college who has received any two of the college awards and is qualified to received a third award. In lieu of the third award, the distinguished Fellow award is given.
The Awards Committee reserves the right to determine the final list of nominees. Members of the PHA Board of Directors are disqualified from being proponents or nominees to any of the awards.
